Registration packets are available in the front office of your neighborhood school.
What to bring when you register your child:
- Original Birth Certificate
- Immunization Record
- Proof of Residence – utility bill, sales contract, or lease agreement (cell phone bill is NOT acceptable)
- Legal papers, if applicable for custody
- Withdrawal form and report card from previous school
- Photo ID for parent/guardian

Open enrollment is the process that allows parents who live in a different school district’s boundaries (other than Laveen Elementary School District) to apply to have their students attend a Laveen school.
- We accept applications each year until May 1, and we base approval on room availability.
- On or before one week after school begins, we will notify the parent or legal guardian in writing as to whether we have accepted, rejected, or placed the application on a waiting list.
- Transportation for the student will be the responsibility of the parent or legal guardian.
- Providing false information on the open enrollment form may result in the application being revoked.
